ANALISIS KECELAKAAN KERJA PADA AKTIVITAS LOADING CYLINDER ARM MENGGUNAKAN METODE ECFA DAN AEB DI PERUSAHAAN DISTRIBUTOR ALAT BERAT

Abstract

Heavy equipment distribution companies have high occupational risk because their operational activities involve mechanical work, heavy material handling, work equipment, and direct interaction between workers, materials, and mechanical devices. Based on company accident data from 2022 to 2025, there were 20 occupational accident cases with fluctuating and recurring trends, including several cases classified as Major Lost Time Injury (Major LTI). This study aims to analyze one occupational accident case during the loading activity of a cylinder arm onto an expedition vehicle in the workshop area using the Event and Causal Factor Analysis (ECFA) and Accident Evolution Barrier (AEB) methods. The research data were obtained from the company’s internal accident investigation documents, accident chronology, observations, and interviews with related personnel. The ECFA results show that the accident evolved through a sequence of interrelated events, starting from manual loading without mechanical lifting aids, absence of toolbox meeting and hazard identification, workers being positioned too close to the material, one worker tripping, loss of grip on the cylinder arm, material shifting forward, and the worker’s index finger being crushed. The AEB analysis indicates several missing and ineffective barriers, particularly mechanical lifting aids, Job Safety Analysis, safe zone arrangement, housekeeping, and job supervision. Recommended controls include the use of mechanical lifting aids, development of heavy material loading procedures, implementation of toolbox meetings and JSA, establishment of exclusion zones, and improvement of work supervision.
