ANALISIS RISIKO KECELAKAAN KERJA PADA PROSES REPARASI KAPAL MENGGUNAKAN METODE FMEA DAN FTA

The ship repair process in shipyards involves various high-risk activities that can lead to occupational
accidents. Workplace accidents in shipyards are a serious issue, as they can affect worker safety,
productivity, and overall company operations. One private shipyard in Surabaya experienced a workplace
accident that resulted in a complete operational shutdown for several weeks. This study aims to identify
occupational risks occurring during the ship repair process, analyze their root causes, and design effective
mitigation strategies. The methods used in this research include FMEA to assess risks based on severity,
occurrence, and detectability, and FTA to identify the root causes of these risks. This research identified 15
occupational accident risks in the ship repair process. The FMEA analysis revealed the 2 risks with the
highest RPN values: workers entangled in mooring ropes and workers involved in explosions. The FTA
analysis identified 9 basic events. The proposed mitigation strategies include direct observation, pre-work
briefings, mandatory PPe usage, workplace inspection, development of SOPs, and equipment checks.
