Abstract

 

This research was conducted at a Stevedoring Company located in Surabaya, engaged in the loading and unloading services of containerized and dry bulk cargo on both international and domestic scales. To support its business activities, there is an Operations Planning and Business Process Department responsible for planning and reviewing business processes to ensure quality assurance in every business process carried out by the Container Stevedoring Company. In all its operational activities, risk is an inseparable part of every activity conducted. Operational risks can arise from non- functional internal processes, human resource errors, system failures, and various external factors. This research employs FMEA and the results indicate that there are 12 indicators, with 1 high-risk indicator, 4 medium-risk indicators, and 7 low-risk indicators. The three risk indicators with the highest RPN values are Equipment Damage during Loading and Unloading with a score of 248.48; Port Competition in the Tanjung Perak area with a score of 124.29; and Cargo/Container Damage with a score of 106.11.
