Abstract

The pre-cast concrete company in Mojokerto is one of the manufacturing companies. The process of making concrete takes place for 24 hours, this results in the machines used in the production process are prone to failures that can hamper the production process. One of the tools that operate 24 hours is the Batching Plant. The failure data from 2022-2024, shows that the Batching Plant is the tool that has the highest number of failures of all the tools in the company. The purpose of this study is to conduct an analysis of component failures in the Batching Plant so that similar events do not recur and can be minimised. This research uses the Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA) methods. The results of the analysis using FMEA, found 58 potential failure modes, then continued to determine critical components using Pareto diagrams and showed 5 critical components, namely, mixing blade, conveyor belt , screw conveyor motor, pressure relief valve, and conveyor belt  drive motor which were used as top events for making FTA. The results of the FTA found the basic causes in the mixing blade component 10 basic events, Fault Tree Analysis 9 basic events, screw conveyor motor 12 basic events, pressure relief valve 9 basic events, conveyor belt  drive motor 11 basic events. Recommendations are given using a control hierarchy that is adjusted to the type of basic cause of each top event.
