Analisis Human Error pada Pekerjaan Forming Perusahaan Beton Pracetak dengan Metode HEART

Abstract

Precast concrete companies are manufacturing companies that produce culverts, gutters, fence panels, wall panels, and so on. Based on work accident reports obtained from the company, unsafe actions dominate as the cause of work accidents that occurred in the period 2021 to 2023. 29% of work accidents due to unsafe actions are caused by human violation factors and 71% are caused by human error factors. The results of interviews with the chief HSE of precast concrete companies state that work accidents due to human error occur mostly in forming work which consists of 3 jobs, namely cutting, bending, and welding of rebar. From the data on the increasing number of work accidents in the company every year, of course, a more in-depth accident investigation is needed to realise zero accident conditions. Therefore, it is necessary to analyse human error in forming work using the Human Error Assessment and Reduction Technique (HEART) method to determine and analyse the probability of human error in order to provide recommendations that can be proposed to reduce the possibility of human error in the forming work of precast concrete companies. Based on the results of the analysis, there are 7 sub tasks from 3 jobs that have high HEP values. The recommendations given are in the form of training for operators, evaluating machine operation techniques, routine monitoring and supervision of the entire work process, and improving work facilities and environments to better support worker health and comfort.
