Penerapan Metode FMEA dan FTA pada Analisis Kegagalan Komponen Engine Side Loader
Abstract
Container handling process (LoLo - Lift On Lift Off) requires specialized equipment that crucial to support these activities. One of the equipment that assists in LoLo operations is the Side Loader (SL), a container handling device with high mobility specifically designed for lifting empty containers. However, there are still many failures in this heavy equipment side loader. The engine is a vital main component for generating power. Careful planning is required to reduce the possibility of engine failures, which can directly impact company operations and assets. Company accident data from 2023 and 2024, various incidents were attributed to failures in the Side Loader (SL) equipment. Using the Failure Mode and Effects Analysis (FMEA) method, this approach will aid in assessing the risk of failures, which will be analyzed for severity, occurrence, and detection. Subsequently, Risk Priority Number (RPN) values will be determined, with the highest RPN indicating the primary failure cause to be investigated. The analysis using FMEA identified 10 sub-components in the engine, with one critical component having an RPN of 63, specifically the accumulator sub-component with the failure mode of battery damage, which will serve as the top event for creating the Fault Tree Analysis (FTA). FTA is a logical and graphical model consisting of various fault combinations depicted using a tree diagram. The FTA results identified 9 basic causes related to the accumulator sub-component battery. The research findings culminated in recommendations based on a hierarchy of controls.
