ANALISIS RISIKO BAHAYA PADA PROSES PANEN/DOFFING DENGAN MENGGUNAKAN METODE FTA (STUDI KASUS: PERUSAHAAN KARUNG PLASTIK)

Abstract

Industrial and technological advances support the entire work process. So that there is an increase in the absorption of labor. One of the industries in manufacturing produces plastic sacks. One of the divisions that play a role is the extruder division which is one of the divisions in a plastic sack manufacturing company. This division has 3 types of machines that work the same way, namely Hagihara machines, Lohia Corp machines, and Starex machines. The process carried out in the extruder division is the process of melting plastic pellets into yarn or plastic yarn. This research was conducted with the aim of analyzing risk using the Fault Tree Analysis (FTA) method, especially in the yarn harvesting process. This is because based on the identification results found high risk in the yarn harvesting process with a total risk assessment of 10. The analysis carried out using the FTA method in this work resulted in 14 basic causes that were connected to each other causing the top event to occur in the form of being pinched by the winder machine and causing a broken hand to occur. The basic cause results of this FTA are given recommendations according to the control hierarchy. The control results obtained are in the form of substitution control by replacing all PPE that has been damaged. Engineering engineering control in the form of installation of exhaust fans. Administrative control in the form of reviewing production targets per day, daily checkups for all workers, routine supervision, job training, work competency certification, implementation of safety briefings, reviewing SOP, socializing SOP, budgeting for PPE procurement, planning maintenance budgets, installing safety signs and controlling PPE the use of safety gloves.
