Making of HIRARC and Management Information System of Work Permit Based on Web and Android (Study Case in Shipbuilding Institute Polytechnic of Surabaya)

Abstract

Shipbuilding Institute  Polytechnic of Surabaya (PPNS) through UPIK3L is committed to implementing SMK3, one of the clauses of which is the existence of a work permit system for high-risk jobs. Therefore it is necessary to apply the work permit system in PPNS especially for production activities. This study aims to conduct hazard identification and risk assessment on production work in PPNS, make procedures and work permit forms for production work at PPNS, and create a work permit management information system for production work at PPNS. The risk of accidents on production work at PPNS can be known if hazard identification and risk assessment are carried out using the Hazard Identification, Risk Assessment and Risk Control (HIRARC) method. Then for high-risk work, a work permit system must be made by making procedures and forms, and making the information management system work. With Web and Android-based information systems, the licensing process is easier and more efficient. After conducting a risk assessment, 18 high-risk jobs were found so that it was required to apply Work Permit. Then the researcher made a procedure and form Work Permit. Making information systems using MySQL, then create an interface using the PHP programming language for the Web and the Java programming language for Android.
