Abstract

FMEA (Failure Mode and Effect Analysis) is a method used to identify how a device, facility or system experiences a failure (failure) and its consequences aftermath. FMEA determines the risk rating represented by the value of RPN (Risk Priority Number). This research of the purposes  to analyze the failure of the stand 26 block mill machine in steel smelting company and determine critical component with pareto diagram. From the data in the field, stand 26 block mill machine were recorded very high. These failures can cause a delay in the production process and work accident. The results of the analysis shows that there are 9 forms of failure with the highest RPN value of 392 in the TC Ring component with the TC Ring broken failure mode.  the Pareto diagram shows that the priority of maintenance or prevention of failure modes that need to be considered are TC Ring, Mandrell, Gear, Bearing, and Locking TC Ring components because the components are at 80% on the cumulative percentage axis of the RPN.
